I have had sililar problems that you have described - but have been able to get them to go away by replacing thermostat, replace radiator, coolant, water pump. I finally replaced the clutch fan at about 170,000 and that eliminated problems when climbing long hills. I would suggest that if the clutch fan has about 150,000 miles - replace it also.
