I wouldn't be so quick to assume the gearbox is dying.  Change the gear oil in the transmission and see if it changes.  While you're under there, check to see if there appears to be an oil leak at the rear of the tranny where the driveshaft goes in. Also check whether the dust shield where the d/shaft goes into the tranny is loose (was on mine, cause a helluva rattle).

If the noise doesn't go away w/ the oil change, see if it changes w/ clutch in/out sitting still (throwout bearing).  If that's not it, it could be bearings in the tranny.  If it only does it in 4th, that's a different bearing than if it does do it in other gears (whether the layshaft is spinning or not).
