Doug:
    Check the fan motor fuse first.  If it is OK and you have a lot of time on the fan motor the brushes may have just worn out and you will need to replace the fan motor.  The other possiblility is that the resistor matrix mouted in/on the heater box behind your glove box has "opened circuited".  Both items are common in the junk yard.  Sort the problems out one at a time, the other problems sound unrelated.
