I have heard of this problem before. Do you have a perfermance style clutch? Some performance clutches really stress the system. They are just too grippy.

Not really that much to do wrong. Just make sure everything is tight and the clutch peddle is set correctly. Bleed the system afterwards.

I have had to replace my clutch before. I think that was the first time the clutch was changed (can't be sure though). The car had around 150,000 miles. I replaced my clutch master when I replaced the clutch.
I havent had any problems with the clutch since.

Start off with the intake or the exhaust. There are a couple of articles in the resources section.
